We are thrilled to welcome you as a patient at High Desert Dental. Your first appointment will be to review your dental and medical history, to update your X-rays and to examine your teeth. The Doctor will discuss a course of treatment based on your individual needs. Following your initial appointment, dental cleanings and other treatment will be scheduled. Should you have existing X-rays (taken within the last 5 years) it is ideal to provide those prior to your first appointment. We look forward to caring for you and your family.
We accept most forms of dental insurance, please check directly with your insurance provider to determine whether we are in or out of network. We do not accept the Oregon Health Plan. For further information please see their website or contact them directly at 503-947-5488.
We do not provide in-office payments plans, however, we do accept CareCredit as a payment option. To apply or for more information follow this link CareCredit.com.
If you have access to a printer, you may save yourself some time on the day of your appointment by printing the following documents, and completing them before your visit. If not please ARRIVE 15 MINUTES before your scheduled appointment time in order to allow for time to complete the forms.
